How to Report an Issue
Welcome to the issue reporting form! Please follow these steps to tell us about the issue you're experiencing.
Step 1: Location of the Issue
This section is all about where the issue is located.
Select Existing Address (if applicable):
- If you are logged in and have previously reported issues, you might see a dropdown menu labeled "Select Address."
- If the issue is at one of your existing locations, choose it from the dropdown. This will automatically fill in the address fields below.
- If there is only one address, the system will prefill it for you.
- If you have multiple addresses but don't want to use them, skip this step, and go to the below to enter a new one.
- If the location of the issue is not in the list, go to the next step to enter a new address.
Enter the Address Manually:
- Street Address: Type the street number and name where the issue is located. (e.g., 123 Main Street). This field is required.
- Apt. #: If the location is an apartment or unit, enter the apartment number here. If it is a house, leave it blank.
- City: Enter the city where the issue is located. This field is required.
- State: Select the state from the dropdown menu. This field is required.
- Zip: Enter the five-digit zip code. This field is required.
Step 2: Describe the Issue
This is where you tell us the details about the problem.
Describe the Issue:
- In the large text box provided, write a clear and detailed description of the issue you're reporting.
- Be as specific as possible.
- Include what happened, where it's happening, and any relevant observations.
- This field is required.
Step 3: Category of the Issue
This section helps us understand what kind of problem you are reporting.
Select Issue Categories:
- Click on the "Select one or many categories here" box.
- Choose the category or categories that best describe the issue. You can select multiple categories if the issue is related to several areas.
- This field is required.
Optional Step: Add Related Images or Videos
This step is helpful, but not mandatory.
Attach Files:
- If you have images or videos related to the issue, you can upload them to help us understand the situation better.
- On large screen size computer, there will be a file pond upload area where you can drag the files to upload.
- On a smaller screen size phone, you can upload files using the default upload way.
- The app support image type: jpeg, png, and gif.
- The app also support video type: mp4, mpeg, and quicktime.
- The maximum file size is 150MB per file.
Final Step: Submit the Issue
- Review: Double-check all the information you've entered to make sure it's accurate.
- Submit: Click the "Submit" button to send your issue report.
- Before submit, the system will check if you are a human by using recaptcha.
- If you are a human, the system will process your information and submit it.
- Otherwise, you will get an error message, and you have to try again.
- If there is an error on the form, an error message will be displayed.
After Submitting:
- You'll see a message confirming that your issue has been reported.
- If you are logged in, you will be redirected to the dashboard.
- If you are not logged in, you will be redirected to a temporary user registration page.
Tips for Reporting Issues Effectively:
- Be Clear and Concise: Get straight to the point when describing the issue.
- Provide Details: The more information you provide, the easier it is for us to understand and resolve the problem.
- Be Specific: Avoid vague descriptions. Instead of saying "something is broken," say "the faucet in the bathroom is leaking."
- Use Visuals: Images and videos are very helpful for demonstrating the issue.
- Mandatory inputs: Street Address, City, State, Zip, Description, and issue Categories are mandatory input.
